CONSULTATION TERMS AND CONDITIONS

These are the terms and conditions on which I will provide consultancy services and supply my professional
advice to you. Please check the box below to indicate that you have read and understand them..

1. MY SERVICES
1.1 I practice as a medical herbalist in accordance with AMH guidelines and code of conduct as set out in the AMH
Constitution, providing herbal medicine services to clients in based upon private consultations. Such consultations
may take place in person or remotely if deemed appropriate.
1.2 Consultations will require you to provide details of all aspects of your current health problems, lifestyle, diet,
medical and family history; it is important that the information that you supply is complete and accurate. I will
collect and hold all such personal data subject to applicable data privacy laws and in accordance with my Privacy
Policy, which is hereby incorporated into the terms.
1.3 Following our consultation(s) (and the supply of any further information required), I will prescribe a personalised
health programme. This may include herbs, supplements and other natural products (or combinations thereof) and
directions for use, together with advice relating to your diet and lifestyle.
1.4 Prescription herbs and emails of my advice will be dispensed to you. I will indicate during our consultation(s) the
approximate timeframe for delivery of these to you. If supply is delayed by events outside my control, then I will
contact you as soon as possible to let you know and will take steps to minimise the effect of the delay.
2. FEES AND PAYMENT
2.1 Fees and payment terms for consultations, the dispensing of herbs and provision of a programme for you to
follow thereafter are set out in my price list in force at the date of booking unless we have agreed another price in
writing.
2.2 You may terminate this contract at any time before I have completed the services, subject to you paying for those services which I have delivered, and a cancellation fee if applicable, laid out in the Patient Agreement. Once a consultation starts, the fees for the full consultation will still apply. You cannot return herbs in whatever form, once dispensed to you.
3. MY LIABILITY
I do not exclude or limit in any way my liability to you where it would be unlawful to do so, including liability for
death or personal injury caused by my negligence; fraud; or breach of your legal rights in relation to any
prescribed products or my services. Subject to that, liability is limited to a maximum of any sum you have paid to
me/ as the cost of my services. I only prescribe products for your individual use; I will not be liable for any loss
of any kind whatsoever if any herbs which I dispense to you are used for commercial or re-sale purposes, or they
are used by any third party.
4. OTHER IMPORTANT TERMS
4.1 These terms are governed by English law and proceedings can only be brought in the English courts.

We are subject to a common law duty of confidentiality. However, there are circumstances where we will share relevant health and care information. These are where:
